North Homes Children & Family Services is a private, non-profit organization dedicated to providing compassionate, community-based mental health services to children, adults, and families across Northern Minnesota. As our Adult Services Program continues to grow, we are looking for a motivated, compassionate Adult Targeted Case Manager who wants to make a meaningful difference in the lives of adults living with serious and persistent mental illness (SPMI). What You'll Do As an Adult Targeted Case Manager, you will work closely with clients to connect them to the services and supports they need to thrive. You will play a key role in coordinating care, advocating for clients, and helping individuals build stronger, more stable lives. Client Support & Advocacy Assist adults with SPMI in accessing medical, mental health, housing, vocational, and community services Help clients identify strengths and areas of need Support clients in achieving goals identified in their Individual Community Support Plan (ICSP) Advocate for clients while promoting independence and self-sufficiency Care Coordination & Communication Collaborate with social workers, probation officers, healthcare providers, and other community partners Maintain professional and effective communication with clients, families, and service providers Coordinate services to ensure clients receive consistent and comprehensive care Assessment & Planning Complete Functional Assessments and develop Individual Community Support Plans Monitor service delivery and ensure services meet client needs Provide referrals and assist clients in navigating complex service systems Documentation & Professional Development Maintain accurate, timely documentation according to agency and licensing requirements Attend required trainings and continuing education opportunities Stay informed on best practices in mental health services Additional Responsibilities Perform other duties as assigned to support program and agency needs Why Join North Homes?
