Description The Guidance Center (TGC) is the Community Mental Health Center and Certified Behavioral Health Clinic serving Leavenworth, Atchison, and Jefferson counties in Kansas. The Guidance Center (TGC) is currently seeking an Adult Targeted Case Manager to be the newest member of our innovative interdisciplinary team. This position provides Community Psychiatric Support and Treatment (CPST) and Targeted Case Management (TCM) services to SPMI and SMI adults, following the strengths model of case management. Evenings Required (1+ days per week after 5 pm). Primary Duties: Under the supervision of a QMHP, the TCM will facilitate the treatment plan process by inviting all involved participants, to include client to develop or update a plan which addresses identifies needs, defines goals and objectives, and action steps which support community living and participation. Enter treatment plan details into electronic records and other documents in a timely manner. Monitor treatment plans to ensure the plan is current, ensure that updates are accomplished before the plan expires, and enlist assistance of supervisor if problems are encountered. Assist in onboarding and care coordination for the Service Members Veterans and Families care team, as needed Provide face-to-face, phone and written communication with caregivers, other service agencies, housing resources, and medical services to assist with meeting client/family needs. Provide assertive outreach directed toward engaging and maintaining client relationships as well as relationships with caregivers Assure that client information is communicated to key staff prior to and after appointments Advocate for and with clients as they seek to achieve their goals and overcome barriers Assist clients or caregivers in obtaining access to needed medical, social, educational, and other services. Transport clients to and from appointments and activities when necessary. QUALIFICATIONS: EDUCATION AND EXPERIENCE: Bachelor’s degree required or be equivalently qualified by work experience or a combination of education and work experience in the human services field with one year of experience substituting for one year of education.
