The role of a Case Manager is to work with adults (ages 21 and up) to identify needs and goals that will strengthen the adult's future. Areas of focus include but are not limited to mental health, physical health, economic needs, education and positive social development. Clients are met with weekly on average and work towards these goals together in the community where the needs are as well as in the home itself. Staff transport clients when needed to make sure important connections to community resources are successful. The combined efforts of the case manager and the informed, client led approach, deliver some of the best outcomes in the field of human service work. Willingness to handle male adult clients Minimum of 4-year degree (related human service degree preferred but not required) Valid Driver's License for past 5 years Ability to commute/relocate: Hagerstown, MD 21740: Reliably commute or planning to relocate before starting work Schedule: Day Shift (Required) - but may require some evenings on a case-to-case basis. Work Location: In person First 2-4 weeks of training will be in our Hagerstown office (Required). Stipend would be provided - to be further discussed upon hire. Responsibilities

The Case Manager works with adults to identify needs and goals that will strengthen their future, focusing on areas such as mental health, physical health, and education. Clients are met weekly to work towards these goals in the community and at home, with staff providing transportation as needed.
