We are seeking a compassionate and confident Advanced Practice Provider & Supervisor who will provide care to our patients and supervise a growing team. This role will provide patient care and leadership to the specialty providers and advanced provider practitioners team who provide care at all our sites. We are looking for a provider and supervisor who is committed to our values and has a passion for increasing health care access and outcomes for underserved communities. Preferably bilingual (English/Spanish) Committed to providing high-quality care to underserved patients in central Orange County and the High Desert, California. Responsible for assessment, diagnosis, and management of healthcare needs, utilizing standardized and evidenced based processes. Work collaboratively with physicians and other health care providers. Lead and supervise a team of Nurse Practitioners & Specialty Physicians This position will report to our Chief Medical Officer. St. Jude Neighborhood Health Centers utilizes EPIC for our Electronic Medical Records (EMR) system. Ambulatory (outpatient) care environment Daytime work schedule 8am to 5pm, with after-hours call for critical labs Position is a (5) five-day work week. Position requires working one Saturday per month with a weekday flexed off. Who We Are. We are a collaborative community practice, with locations in Fullerton, Anaheim, Orange, and the High Desert. Our health centers use a team-based model of care that includes family medicine, obstetrics, women’s health, oral and vision care, preventative and educational health, mental health, patient advocacy, case management, and community health. This position will be required to work one Saturday per month for which a weekday is flexed off. You would be joining a team of family physicians, nurse practitioners, RNs, pediatricians, OB-GYNs, dentists, specialists (optometrist, podiatrist), licensed clinical social workers, mental health professionals, case managers, health educators, registered dietitians, LVNs, and promotoras. Provide direct preventative and acute patient care. Shows the ability to assess and interpret data about the patient’s status to identify specific needs and provide the care needed. Ensure timely and complete documentation of patient care, review and act upon reports, results and electronic tasks. Submit correct billing codes for each patient encounter. Flexible and adaptable, moving practice location as necessary to address patient access challenges. Patient care accounts for approximately 60% of the time of this position. Lead and supervise advance practice providers across multiple sites in Southern California in achieving organization goals and objectives in a manner reflective of our Core Values and Mission. Develop, mentor and foster accountability among our Nurse Practitioner and Specialty providers and guide them to become successful providers through coaching and counseling, professional development, annual reviews, huddles, and being present. Mentor advanced providers in meeting and exceeding quality and productivity goals. Leadership duties account for approximately 40% of the time of this position. Partner with operational leadership in the development, maintenance and implementation of standardized patient procedures, scope of care, clinical policies and new clinical initiatives and services ensuring high-quality, safe patient care. Partner with the CMO and Medical Services Manager to develop and deliver high quality patient care resulting in a high level of quality and patient satisfaction. Utilize community resources to optimize patient care and referral. Using a team-based model of care, collaborates with and supports other providers and caregivers in the wide network to effectively improve the overall health of our communities. Consult with the Chief Medical Officer in cases outside of scope of practice. Adheres to department and organizational programs. Treats all information and data within the scope of the position with complete confidentiality and security. Requirements Job Requirements: Education: MSN-NP/DNP, or MSPA/DPA License / Certifications: Licensed NP/PA in the State of California. DEA certified. Provider level basic life support (BLS) from AHA approved program. Valid California State ID or Driver License Experience: 3-5 years of experience providing direct patient care. Demonstrated leading and supervising others. Knowledge/Skills/Abilities: Ability to interface effectively with staff, volunteers, and other physicians/providers. Ability to obtain and maintain credentials at St. Jude Neighborhood Health Centers, Medi-Cal, and Medi-Cal MCOs. Willingness to use a translator or translation services to best serve patient treatment needs. Proficient in computer programs and systems, including email, internet, and Microsoft Office suite. Expertise in applying evidence-based medicine to treat and diagnosis. Demonstrated ability in applying methods of scientific inquiry and exercising clinical judgment in challenging situations. Exemplar communication and leadership skills, including active listening, taking initiative, following guidance, and setting examples for excellent care of patients. Commitment to continuous learning and professional development, remaining current on medical practices and professional credentialing and licensure necessary for continuous employment with St. Jude Neighborhood Health Centers.
