The Advanced Clinical Practitioner (ACP) role at Oxleas NHS Foundation Trust is an autonomous senior clinical position focused on providing advanced clinical care and leadership for the tri-borough Depot Medication Service. This service offers a safe and efficient step-down from intensive case management for psychosis teams, allowing service users to receive depot injections, annual mental health reviews, physical health checks, and wellbeing services. The ACP will be an Independent Prescriber whose scope of practice meets the needs of individuals within the psychosis pathway. The role involves leading the delivery of high-quality, efficient, safe, and evidence-based care, ensuring consistently high standards of practice and clinical excellence. The ACP will also promote population health, improve personal outcomes, contribute to service evaluation, and lead research and quality improvement initiatives. Additionally, the ACP will provide clinical supervision and support the development of staff within the Depot Medication Service.
The ACP will lead the delivery of specialist assessments, interventions, and treatment plans for service users within the Depot Medication Service. This includes contributing to the prescribing provision through supportive dedicated non-medical prescribing. The ACP will lead innovations and improvements to standards of care by developing care pathways, providing clinical supervision, conducting clinical audits, and supporting professional colleagues through teaching and skilled clinical leadership. The ACP will act in accordance with professional regulatory standards and be accountable for their clinical practice, professional actions, and omissions at all times. They will retain clinical competency, ensuring their skills are up-to-date and relevant to the role. The ACP will also lead the implementation of research into practice and participate in relevant audits and research studies to aid the development of the evidence base and contribute to clinical governance. Advanced problem-solving and decision-making skills are essential, as the ACP will contribute to long-term planning for future developments within the immediate clinical area, Trust, and local health economy. The ACP will provide clinical oversight and leadership for the tri-borough Depot Medication Service, ensuring the delivery of a safe, organized, and holistic service for service users. They will lead the Depot Service Nurses and oversee service development and evaluation, ensuring the delivery of a high-quality evidence-based service. The ACP will demonstrate a deep and systematic knowledge and understanding of local care systems, collaborating with and referring to them independently when required. As the clinical lead, the ACP will deliver specialist-specific interventions, working in collaboration with the person, their family, carers, and a range of professionals and agencies. The ACP will exemplify leadership, resilience, and determination, managing situations that are unfamiliar, uncertain, complex, or unpredictable, and seeking to build confidence in others. They will receive, lead, deliver, and exemplify a culture of critically reflective clinical supervision.
