Overview
An ACP in this role is an experienced, master’s-level healthcare professional (e.g., nurse, paramedic, pharmacist, or other allied health professional) operating autonomously within primary care. They support the general practice team by managing complex cases, providing leadership, and contributing across clinical, educational, and research domains. (Primary Care Careers, gpworld.co.uk)

Key Responsibilities Clinical Practice

  • Conduct comprehensive patient assessments: history, examination, clinical reasoning, investigation, diagnosis, and management.
  • Autonomously manage a diverse caseload, including acute, chronic, undifferentiated, and complex conditions.
  • Prescribe medications where qualified (Independent Prescriber or PGD); safely initiate, review, and adjust treatment plans; conduct medication reviews and domiciliary visits as needed.
  • Use EMIS for accurate documentation, coding, and data extraction.
  • Conduct consultations face-to-face, via telephone/video, and undertake home or care-home visits.
  • Escalate emergencies appropriately and manage risk within scope of practice.

Leadership & Collaboration

  • Support and educate other clinicians
  • Contribute to practice policies, performance reviews, cost-effective prescribing, and adherence to NICE/CQC standards. (Rest Less, NHS Jobs)

  • Maintain a Personal Development Plan (PDP) and CPD portfolio according to regulatory bodies

Person Specification

ESSENTIAL QUALIFICATIONS & EXPERIENCE:

  • Registered practitioner (NMC, HCPC, GPhC).
  • Master’s degree or post-graduate level 7 qualification in Advanced Clinical Practice
  • Independent Prescriber qualification or PGD (depending on professional background).

SKILLS & COMPETENCIES:

  • Advanced clinical assessment, diagnostic, and decision-making skills.
  • Proficient in using clinical systems and documentation.
  • Strong leadership, communication, and interpersonal skills.
  • Ability to work autonomously while recognising limitations.
