A vacancy has become available for an Advanced Frailty Practitioner within the East Dunbartonshire HSCP, the post will sit as part of the Community Rehab Team.
As part of the GGC wide network of Advanced Frailty Practitioner posts, this post will be the cornerstone in developing and delivering an enhanced frailty response, supporting those with frailty to maximise their independence and wellbeing within their local communities, prevention avoidable hospital admissions and early supported discharge to minimise the known adverse impact of hospital admission.
East Dunbartonshire welcome applications from experienced and innovative practitioners with an appetite for service improvement and self-development, from across the NMAHP and pharmacy workforce. The advanced practice Band 7 post will work across the primary secondary interface, with Frailty Practitioners aligned within acute front doors and in each HSCP area. Critical to the success of these roles will be facilitating and enabling communication between and within multidisciplinary teams across acute and community services. The frailty practitioners across GGC will work as a virtual network and enable high level joint clinical decision making, with regard to right care, right place and right time to maximise patient outcomes, preventing avoidable hospital presentation and admission, and supporting discharge without delays.
An agreed education and governance framework is in place, with the postholder being required to complete specific Masters level qualifications.
