We are seeking a highly motivated and experienced Frailty Practitioner to join our expanding Frailty Service in the North Sector.
This is a key role within a multidisciplinary team, supporting the delivery of high-quality, person-centred care to older adults living with frailty. The Frailty Service in the North Sector is a dynamic and evolving model of care designed to meet the complex needs of older adults living with frailty. Based at Glasgow Royal Infirmary, the service integrates early identification using the HIS Frailty Assessment Tool, rapid access to Comprehensive Geriatric Assessment (CGA), and coordinated multidisciplinary input. The service has proudly demonstrated measurable improvements in patient outcomes, reduced length of stay and enhanced discharge planning. The team works closely with health and social care partners to deliver person-centred care, and is committed to continuous improvement, education, and innovation in frailty management.
As part of the GGC wide network of Advanced Frailty Practitioner posts, this post will be the cornerstone in developing and delivering this innovative service, supporting those with frailty to maximise their independence and wellbeing within their local communities and preventing avoidable hospital admissions.
GGC are committed to improving and connecting integrated frailty pathways, and significant investment supports the delivery of this initiative and advanced frailty practitioner posts. North Sector, Older Peoples Service welcome applications from experienced and innovative practitioners with an appetite for service improvement and self-development, from across the NMAHP and pharmacy workforce. These advanced practice Band 7 posts, will work across the primary/secondary interface, with Frailty Practitioners aligned within Glasgow Royal Infirmary front door/receiving area while maintaining close links with HSCP colleagues in local area. Critical to the success of these roles will be facilitating and enabling communication between and within multidisciplinary teams across acute and community services. The frailty practitioners will work as a virtual network and enable high level joint clinical decision making, with regard to right care, right place and right time to maximise patient outcomes, preventing avoidable hospital presentation and admission, and supporting discharge without delays. An agreed education and governance framework is in place, with all post holders being required to complete specific Masters level qualifications. All applicants should be registered with the appropriate body and have a full drivers licence.
