Norfolk Primary Care CIC (NPC) is an established Primary Care provider of clinical services. We are a key provider of Primary Care services in the Norfolk & Waveney system working with the local GP practices.
NPC has been sub-contracted to provide a Primary Care / General Practice type service within the Emergency Departments (ED) of the local Acute Hospitals. This comprises of a team of clinicians who are GP’s, Advanced Nurse Practitioners or Advanced Clinical Practitioners working across the various locations. The staff provide additional Primary Care support to the ED by streaming patients away from the Front door into a Service capable of supporting demand.
The aim of the GPFD is to reduce the number of avoidable attendances seen within ED, to direct the patients to appropriate services and free up ED staff to see appropriate emergency patients.

JOB SUMMARY

You will be an experienced Primary Care clinician, who will be acting autonomously within their professional boundaries and competencies. You will identify appropriate patients to attend the service based on current inclusion and exclusion criteria, as well as your clinical acumen to provide appropriate care for the presenting patient, from initial history taking, clinical assessment, diagnosis, treatment, referral (where required), evaluation, and discharge from our service.
You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for the patients within GPFD. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the ED and Acute Hospitals to ensure patient journeys are appropriate and timely.
In order to work at this level, those registered with the NMC / HCPC, requirements for Advanced Practice must be met.

Responsibilities

KEY RESPONSIBILITIES:

  • Identify patients appropriate to the service based on initial conversation and excellent communication skills.
  • Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with simple or complex, on the day needs.
  • Proactively identify, diagnose and identify treatment plans for patients at risk of developing a long-term condition (as appropriate).
  • Refer patients for investigation for specialist opinion, where appropriate.
  • Diagnose and manage both acute and chronic conditions with an acute presentation, integrating both drug and non-drug based treatment methods into a management plan.
  • Work with patients in order to support compliance with and adherence to prescribed treatments.
  • Ensure provision of continuity of care including safety netting and suggest follow up, as required.
  • Take advice, guidance and instruction from the registered GP, when and if required.
  • Prescribe safe, effective and appropriate medication as defined by current legislative framework and local formularies.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimes, side-effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Escalate to the Acute Hospital staff as required.
  • Refer patients to other health care professionals as appropriate for clinical needs and in accordance with local framework/guidelines if indicated.
  • Meet the needs of patients presenting for opportunistic long term condition management or assessment.
  • Be able to undertake general telephone triage / first contact assessment.
  • To manage own clinical workload within the GPFD Service, responding effectively to patient need.
  • Adhere to the NMC / HCPC Code of Professional Conduct at all times.

OTHER DUTIES

  • Any other duties may be considered, as agreed with the Service or Clinical Lead to meet the needs of the organisation, and may involve travel to other sites within the organisation.
  • You must hold a full drivers licence with access to a vehicle to use for purposes of travel to and from Acute hospital sites. This will involve having the appropriate insurance to use your vehicle for business use.
