JOB SUMMARY

We are looking for a motivated and caring Advanced Nurse Practitioner to join our friendly GP practice team. This is an exciting opportunity to use your clinical skills in a hybrid general practice and patient home based environment.
You’ll be part of a close-knit, multidisciplinary team, helping us to provide safe, high quality care both in the surgery and out in the community. As a valued member of the team, you’ll also carry out home visits and care home rounds, supporting patients in their own environment and making a real difference to their comfort, independence, and quality of life.

WHO WE ARE LOOKING FOR

  • Registered Advanced Nurse with NMC registration
  • Experience in an urgent care or primary care setting is desirable
  • Independent prescriber or willingness to work towards it (desirable)
  • Excellent clinical, communication, and decision-making skills
  • Ability to work independently and as part of a multi-disciplinary team
  • Confident and efficient using Microsoft Office and IT based patient clinical systems (EMIS, Docman, Accurx is preferable)
  • Current DBS check Enhanced clearance
  • Must have access to own vehicle and relevant up to date business insurance
Responsibilities
  • Working autonomously but with full clinical support, you will manage patients presenting with a wide range of minor illness and injury, as well as long-term conditions where appropriate
  • You will assess, diagnose, treat, refer or signpost patients who attend the surgery
  • Provide clinical assessment, diagnosis, treatment, and evaluation of patients
  • Work collaboratively with GPs, nurses, pharmacy Technician and admin team
  • Support the management of long-term conditions and care planning
  • Support and participate in shared learning across the practice
