LEAD ON COMPLEX CASES THAT CONNECT CARE, HEALTH AND HOUSING TO HELP RESIDENTS LIVE WELL AND INDEPENDENTLY

As an Advanced Practitioner, you’ll lead innovative and person-centred solutions that improve housing and health outcomes for autistic adults and adults with learning disabilities. It’s a unique opportunity to use your professional expertise to make a real difference – working across services, building relationships and supporting people to live well in homes that meet their needs.

WHAT YOU’LL BRING

You’ll be a qualified, registered social worker with significant post-qualifying experience, ideally within complex case management and housing-related support. You’ll bring strong professional judgement and the confidence to work collaboratively across sectors – from social care and housing to health and voluntary services.
Your approach will be strengths-based and person-centred, with a clear focus on improving outcomes and reducing inequalities. You’ll understand how poor housing can impact physical and mental health, and know how to navigate systems to secure better solutions. You’ll also bring experience in staff or student supervision, and be confident using case management and data systems in line with data protection legislation.

Responsibilities

You’ll be the link between adult social care, public health and housing services – identifying where unsuitable housing is affecting residents’ wellbeing, and helping to find solutions that enable people to thrive. Focusing on complex cases, you’ll work across the bi-borough (Westminster and Kensington and Chelsea), helping secure accommodation that promotes stability, prevents hospital admissions and improves long-term outcomes.
You’ll spend much of your time out in the community, working closely with autistic adults and adults with learning disabilities, as well as their families and support networks. Your ability to build trust and understand what matters to each individual will be central to your success. You’ll help co-design support plans, assist during accommodation moves, and connect people to local groups and services to support their integration and wellbeing.
Alongside this, you’ll analyse data, influence service improvements and make sure housing providers are responsive to the specific needs of the people you support. You’ll also represent the service at key meetings and forums, working with colleagues and partners to improve outcomes and raise the profile of this vital work.
