The Adventure-based Case Manager – Community & Family Support Services (CFSS) is central to the coordination and delivery of services to youth/young adults/adults(YYA) involved with the Department of Developmental Services (DDS), Department of Children & Families (DCF), Department of Mental Health (DMH), and the Department of Public Health (DPH) in Adventure-Based, Experiential Learning (AB/EL) Programming. This position works with the participant and their family (as indicated), and/or treatment team to identify their needs and connect them to community resources to address those needs. The Case Manager meets regularly with each young person and family (as indicated) to ensure progress toward goals on the treatment/action plan, to reduce barriers to success, and to evaluate progress at least every 90 days.
The qualified candidate must demonstrate an understanding of or readiness to learn the AB/EL model. This position must demonstrate an understanding of impact of Adverse Childhood Experiences on family systems; substance use or addiction; mental health and trauma; cultural identity; gender identity; high risk behaviors and the impact of collateral trauma and marginalization. Group facilitation and strong relationship-building skills required. Excellent communication and computer skills required. Valid driver’s license, reliable and safe transportation required.
The qualified candidate will have a Bachelor’s or higher degree in Human Services, Counseling, Social Work or closely related field and at last one year of case management experience. Excellent communication and computer skills required. This is a full-time position located in Central Massachusetts and will include community-based travel and implementation. Work schedule must accommodate the needs of the participants and program delivery schedule. Bilingual/Bicultural candidates are encouraged to apply.
