POSITION DESCRIPTION:

The African American Elders Program Director is responsible for actualizing the mission of the African- American Elders Program. The mission is to improve the quality of life of isolated, frail, and low-income African-American elders in King County promoting accessible, high quality, culturally appropriate and comprehensive services. The AAEP identifies isolated African-American elders through outreach and assists them with access to social and health services through ongoing case management, support and referrals. The Program Director must track effectiveness of services through statistical analysis, consumer feedback, and community surveys. This position oversees a program assistant, care coordinators, counselors and nurse(s). This position is responsible for a budget of $1.9 million.

YEARS OF EXPERIENCE/EDUCATION

  • Bachelor’s Degree Human Services, Social Work, Business Administration, Political Science or a closely related field with a minimum of five years of management experience.
  • Minimum 3 years’ experience in the field of geriatrics
  • Preferred: Master’s Degree in Social Work or Nursing and experience working in a social services program that services African-Americans.

POSITION QUALIFICATIONS AND REQUIRED SKILLS

  • A passion for and experience working with an aging population.
  • Success in a governmental or non-profit organization with a proven track record of achieving results in program development.
  • Understand the principles of social service programs and administration.
  • Advanced knowledge of public health and geriatric issues of older adults.
  • Federal, state, and local laws, rules and regulations governing client care.
  • Case worker objectives, principles and methods.
  • Principles and practices of employee supervising and staff development.
  • Working skills in Microsoft Suite, Adobe, and data base management/reporting systems.
  • Excellent communication and interpersonal skills.
  • Strong organizational and time management skills with the ability to work within systems.
  • Ability to work as part of a team or independently.
  • Previous work history in senior services.
  • Demonstrated ability to work with a diverse workforce, to include knowledge of culturally relevant services to African-Americans.Car with valid Driver’s License and automobile insurance.

Responsibilities

PROGRAM PURPOSE:

There are great needs in the African American elder population for these programs to offer resources and support to frail elders. This gap is due to historical institutional racism in the health care system, lack of cultural relevant services and the lack of outreach to the African American community, specifically community resources African American elders’ use and trust. The core objective of the AAEP Program is to connect programs with frail African Americas through partnerships with community resources, to identity contact and educate elders and their families and support systems about available programs
The African American Elders Program serves African-American elders in Central and Southeast Seattle and South King County. The program identifies frail, isolated and hard-to-serve African Americans and assists them in accessing and receiving needed social and health services through ongoing case management, support, referrals, and prevent premature institutionalization.
