Key Responsibilities: Collaborate to provide management and delivery of patient care plans Meet critical deadlines for charting Self-manage time and schedule Demonstrate experience with tablets, mobile phones and EMR software Shift Details: Schedule: Monday – Thursday 5p-8a Travel: Local travel required Requirements qualifications required Associate degree or higher Minimum of 2yrs Hospice Experience 1+ years of experience in ANY of the following: Electronic Medical Record (EMR) Other EMR TruChart EHR Must Have a Valid Registered Nurse (RN) in the state of Pennsylvania (PA) Must have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all 3) Are you okay with the required travel for this position?
