Facilitate, oversee, and coordinate appropriate and continuous aftercare counseling. Facilities group therapy. Identifying resources needed for continued recovery following discharge. Completing progress notes and entering all appropriate aftercare planning interactions, including contact with referring professionals, into the internal database. Verifying authorization to communicate with referring professionals or programs with the parent/legal guardian prior to discharge. Requirements Bachelor’s degree in psychology/human services required. Master’s degree in Human Services field preferred. Experience working with women with eating disorders and discharge planning preferred. Hours: 40 hours per week - Monday-Friday
Responsibilities
The Aftercare Coordinator works with a multidisciplinary team to facilitate and oversee aftercare counseling and group therapy. They are responsible for identifying resources for continued recovery and documenting interactions in the internal database.
