  PURPOSE OF ROLE The Aftercare Coordinator is responsible for supporting Camp Hope alumni as they transition from the Camp Hope Program into independent living, employment, education, and community reintegration. This role ensures continuity of care, sustained recovery support, and reduced risk of relapse, homelessness, or crisis through structured follow-up, mentorship, case management, and connection to community-based resources. The Aftercare Coordinator serves as a key bridge between residential programming and long-term stability, reinforcing Camp Hope’s mission of healing, hope, and sustainable recovery for combat veterans. CORE RESPONSIBILITIES A. Alumni Follow-Up & Engagement * Conduct monthly follow-ups with past program participants for a minimum of 12 months post-discharge. * Maintain consistent communication through phone, text, email, and scheduled check-ins. * Track alumni progress related to housing stability, employment, education, sobriety, and mental health. * Identify early warning signs of relapse, disengagement, or instability and initiate timely interventions. B. Mentorship & Transition Support * Provide structured mentorship to residents, with particular focus on the first 6 months following program completion. * Support veterans in navigating identity shifts from interim housing to independent living. * Reinforce recovery principles, accountability, and healthy coping strategies. * Coordinate with peer mentors and program staff to ensure alignment of support strategies. C. Case Management & Resource Navigation * Assist veterans with navigation to community-based resources, including: * Housing and sober living options * Employment and vocational training * VA and non-VA healthcare services * Mental health and substance use treatment * Legal, financial, and benefits assistance * Develop individualized aftercare plans based on veteran needs and goals. * Serve as a liaison with external providers, agencies, and partner organizations. D. Documentation, Notes & Data Collection * Maintain accurate, timely, and professional case notes and records in designated data systems. * Ensure proper documentation of follow-ups, referrals, interventions, and outcomes. * Support data collection efforts tied to grants, reporting requirements, and program evaluation. * Assist with tracking alumni outcomes to support continuous quality improvement. E. Crisis Management & Risk Mitigation * Respond to alumni crises, including relapse risk, mental health emergencies, housing instability, or safety concerns. * Coordinate with internal leadership, clinical partners, emergency services, and community providers as appropriate. * Participate in crisis de-escalation, safety planning, and referral to higher levels of care when needed. * Document all crisis interventions in accordance with organizational policy. QUALIFICATIONS & COMPETENCIES Minimum Qualifications * High school diploma or equivalent required. * Experience working with veterans, individuals in recovery, or vulnerable populations preferred. * Strong interpersonal communication and mentorship skills. * Ability to maintain accurate documentation and manage caseload responsibilities. * Basic computer proficiency and familiarity with data entry systems. Preferred Qualifications * Associate’s or Bachelor’s degree in Social Work, Counseling, Psychology, Human Services, or related field. * Prior experience in case management, peer support, transitional housing, or recovery programs. * Understanding of PTSD, substance use disorders, and veteran reintegration challenges. * Experience working with VA systems or community-based veteran resources. SALARY  * A portion of this position’s compensation is supported by grant funding. While the organization actively seeks ongoing funding to sustain all programs and roles, adjustments to funding streams may require modification to salary allocations, duties, or position structure. The organization will make reasonable efforts to identify alternative funding or role alignment when feasible. * Starting Compensation: $15.00 per hour.  Upon successful completion of the 90-day introductory period, including satisfactory performance and continued organizational need, the position is eligible for an adjustment to $20.00 per hour. Compensation adjustments are subject to performance evaluation, funding availability, and leadership approval.
