The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region. The focus of IUIH Aged Care team is to work with the local indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in home care services and eligible aged care clients. About the Role? As Aged Care Assessment Manager, your role is to participate in the effective implementation and operational delivery of IUIH's Aged Care Assessment Service. You will be responsible for supporting the assessment workforce to demonstrate quality and performance outcomes. You will ensure client experience is world class and our reputation within the industry is positive. To achieve this, you will participate in meeting all reporting and mandated requirements outlined in departmental funding agreements and ensure that we deliver excellent client outcomes through exemplary service delivery. You will provide exceptional leadership to the Assessment teams. Your day to day duties may include: Monitor the team’s assessments and support planning through formal and informal auditing processes, ensuring they align with KPI requirements. Provide mentoring and coaching to ensure assessments are delivered within IUIH and Departmental Quality Frameworks. Liaise with external stakeholders including networking and emerging stakeholders. Ensure a robust system of clinical governance, including clinical support, incident reporting, risk management and consumer feedback and engagement. Maintain expert knowledge and currency for Single Assessment Service (SAS) programs being delivered, ensuring comprehensive knowledge of the program, relevant legislation and the deliverables as outlined in the related funding agreements. About You? Tertiary qualification in a health-related discipline directly related to health, aged care or related specialist area. Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or part of/eligible to be apart of a relevant professional association. Current Criminal History Check. Current C Class Drivers Licence (QLD). Current Disability Worker Screening Check (Qld Yellow Card). Ability to travel as required to provide leadership to the remote workforce. Desirable, but not mandatory: Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities At least one year's Aged Care Needs Assessment experience is preferred.
