As an Aged Care Assessor you’ll be responsible to undertake face to face assessments using the Integrated Assessment Tool (IAT) to create support plans and eligibility for Commonwealth Home Support Program (CHSP). This assessment works with the client to assess their needs, goals and preferences focussing on a wellness and reablement approach to build their independence. The position will operate according to relevant legislation, Barwon Health policies and department guidelines.
