Responsibilities

ABOUT THE ROLE

As our Registered Nurse/Client Services Manager for our Home Care Package program, you will work collaboratively with our Allied Health Services, Social Support, and external partnered brokered services. You will guide and assist our clients, their carers and representatives with the development and implementation of care plans that are flexible, individualised and appropriate.

YOUR MAIN RESPONSIBILITIES WILL INCLUDE THE FOLLOWING:

  • Responding to inbound calls from potential new customers about the services we provide and conducting assessments and reviews when there is a new client intake, establishing home care agreements, care plans, budgets and connecting our clients with local services and community resources.
  • Guide our new clients every step of the way during their onboarding process providing ongoing support and ensuring person centred care planning, enabling personal preferences and independence at home.
  • Manage a caseload of clients in both our Home Care Package and NDIS programs for high intensity needs.
  • Interact with Carer’s Network team members, stakeholders, partnered service providers.
  • Ensure that all coordinating services and compliance are within business guidelines and meet quality standards.
  • Ensure exceptional high standards of report writing/case notes are all up to date and any concerns regarding your client are proactively managed.
  • Provide clinical support and guidance to the Care Managers (non-clinical) and play an integral role in ensuring they are provided with adequate supervision.
  • Conduct annual reviews for all clients, revising care plans, goals, service provisions to quality standards and budget expenditure and maximisation.
  • Be an effective communicator, with excellent interpersonal skills, great team skills and be able to work within a multidisciplinary team.
