About The Role: Bliss Nursing is looking for dedicated and caring Aged Care Workers / Disability Support Workers to join our professional team to provide person-centred support to older people and individuals living with disabilities, enabling them to live safely, comfortably and as independently as possible within their homes and communities across the following tasks:

• Deliver personal care assistance, including bathing, dressing, personal hygiene, grooming, and mobility support.
• Provide practical assistance with everyday household tasks such as cleaning, laundry, and meal preparation.
• Support clients to attend medical appointments, social outings, shopping, and community-based activities.
• Encourage clients to build confidence, develop daily living skills, and achieve their individual goals.
• Assist with medication prompts or administration in accordance with organisational policies and relevant training.
• Foster positive relationships that promote dignity, respect, independence, and individual choice.
• Accurately complete client documentation, progress notes, and incident reports in a timely manner.
• Follow individual support plans while maintaining privacy, confidentiality, and cultural sensitivity.
• Liaise effectively with families, allied health professionals, care coordinators, and other members of the multidisciplinary team.

Requirements: The ideal candidate will hold a Certificate III or IV in Individual Support (Ageing or Disability), or a relevant higher qualification in related disciplines with 1 year of experience in a similar role.
