Responsibilities

ROLE QUALIFICATIONS

  • Driver’s Licence and Vehicle
  • Minimum requirement Certificate IV in Aged Care /Individual Support/Disability or equivalent
  • Completion of Infection Prevention and Control Training
  • Medication Assistance Competency (where applicable)
  • Manual Handling Certificate
  • Current Basic Life Support (BLS) Certificate
  • Holds current CPR and First Aid Certificate
  • Hand Hygiene Certificate
  • Possesses a NDIS Worker Screening Check
  • Not more than 3 months Police Check
  • Working with Children check (where applicable)
  • Immunisation Record ( Covid Vaccine/Yearly Flu Vaccine or other required immunisation Record)
  • Passport
  • Working rights

ROLES AND RESPONSIBILITIES

  • Assist with daily living activities such as bathing, dressing, grooming, and toileting.
  • Encourage and assist clients to achieve maximum independence in their daily activities.
  • Ensure timely and accurate administration of prescribed medications.
  • Develop and maintain trusting relationships with clients.
  • Engage in meaningful activities and conversations to support emotional well-being and help clients stay connected with family and friends.
  • Observe and report any changes in clients’ health conditions.
  • Help clients with mobility aids and exercises to maintain physical health.
  • Collaborate with Community RNs and other health professionals to ensure comprehensive care.
  • Assist with cleaning, laundry, and maintaining a safe living environment.
  • Plan and prepare nutritious meals according to clients’ dietary needs and preferences.
  • Help with shopping, errands, and other household chores.
  • Accurately document care provided and clients’ progress.
  • Communicate any concerns or incidents to the Community RN or Coordinator promptly.
  • Adhere to individualized care plans and update them as necessary.
  • Participate in mandatory training sessions and continuous education opportunities.
  • Share suggestions and feedback to improve care quality and client satisfaction.

ROLE ADHERENCE

  • Generate and maintain a trusting relationship with clients which includes protecting confidentiality, privacy, individual choice, and decision making.
  • Implements a focused practice ensuring diversity and equity in service delivery, accommodating any special, cultural, or linguistic needs that have been identified.
  • Assists clients to achieve maximum independence by meeting needs including personal, emotional, psychological, cultural, and spiritual needs.
  • Communicate effectively and appropriately with client, their family, friends and allied health associated professional workers by: Respecting privacy, responding in a timely manner, following the client’s decision, appropriate referral, assisting with clients complaints
  • Communicate effectively and appropriately with colleagues and managers by: Maintaining open lines of communication and respecting the chain of command
  • Adhere to Nursing Experts Services Policies and Procedures based on Australian Government standards
