Description JOB SUMMARY The LVN Supervisor, Ambulatory Case Management provides day-to-day leadership and operational oversight for clinical care coordination staff. This role supports the delivery of high-quality, patient-centered care management services by overseeing workflows, assisting with training, monitoring documentation, coordinating assignments, and ensuring departmental and regulatory compliance. Within the LVN scope of practice, the LVN Supervisor performs delegated clinical functions, conducts structured assessments, provides patient education, supports transitional care activities, and helps ensure continuity of care across ambulatory settings. The LVN Supervisor collaborates with managers for escalated or complex clinical decisions. Requirements MINIMUM & PREFERRED QUALIFICATIONS Education/Training Minimum: Graduate of an accredited Licensed Vocational Nurse Program. Preferred: Additional coursework or certification in care coordination, managed care, or population health. Experience Minimum: 2+ years of inpatient review experience within a managed care organization or healthcare facility. Preferred: Previous leadership or care management supervisory experience. Any combination of educational and work experience that would be equivalent to the stated minimum requirements would qualify for consideration of this position. Certification(s) Current State LVN License Current BLS Certificate Skills, Knowledge & Abilities · Strong knowledge of managed care principles and healthcare delivery systems. · Ability to type 40 words per minute with accuracy. · Proficient in Microsoft Office (Word, Excel, Outlook, Access, PowerPoint). · Excellent written and verbal communication skills. · Ability to analyze clinical data, apply criteria, and support program compliance. · Strong organizational skills and the ability to manage multiple priorities. · Demonstrated ability to lead, train, and support staff in a patient-centered environment.
