Key Responsibilities Assist Legacy patients and community members in identifying and addressing barriers to receiving healthcare services. Provide health education and screenings with a goal of increasing health outcomes and access to care. Engage in health fairs, community meetings, and presentations to increase community reach. Assist in the assessment of community health needs through nationally recognized social determinants of health screening tools, including PRAPARE and the Health-Related Social Needs Screening Tool. Utilize a database of assistance organizations to connect individuals with identified social determinants of health barriers to available resources. Provide coordination and support to patients accessing community resources and services. Perform other duties as assigned. Connect community members and patients to primary and preventative health care services. Connect community members to eligibility services. Educate the community about all health care services provided by Legacy. Represent Legacy's AmeriCorps Program by wearing the AmeriCorps logo daily. Assist in disaster response and recovery activities as required. Minimum Qualifications High School diploma or GED is required. Embark on this journey with your educational foundation!
