Description

To meet regulatory, compliance and quality standards of practice, and to support members of the Administrative/Leadership/Quality Team/Volunteer & Berevement, the PSDA facilitates multiple multi-faceted operational projects within the agency; requiring the collection, maintenance, aggregation, analysis and subsequent reporting of all data. This role also performs as a stakeholder within the technical support of EHR; facilitating implementation and operational processes in conjunction with all other applicable departments.

Professional Duties
- Develop and maintain data collection systems for the operational activities of the agency including the QAPI Program, as directed by the Leadership Team.

- Perform data processing and aggregation for QAPI activities, as directed by the Quality Manager/Leadership Team.

- Generate reports and graphics for the purposes of presenting results of all operational activities.

- Participation in task-force related to planning, coordination and implementing of EHR projects.

- Work collaboratively with Leadership to produce EHR generated reports, including productivity, scheduling, census etc.

- Participate in the EHR training of staff.

- Coordinate and communicate EHR system releases, to ensure all end users are prepared for updated functionality.

- Act as a liaison between end users and EHR support services in conjunction with all other applicable departments.

- In conjunction with Leadership, communicate project status and deliverables; following state and federal Conditions of Participation and regulations, and agency policies and procedures.

- Create and maintain data tracking/analysis tools, documents and materials related to QAPI, Staff Education, documentation and CMS reporting.

- Reconcile monthly services with Finance Department, report to Leadership and external authorities as applicable.

- Participate in educational activities related to performance improvement activities and regulatory changes, including the development and maintenance of department training materials.

- Assist in the management of operational forms including, but not limited to; creating, updating, ordering, printing, and stock control, while supporting services of HIS e.g. scanning and filing.

- CMS mandated service data entry, including but not limited to: discipline service entry and medications. Utilization service data entry, including care coordination, travel, pto, meetings etc to enable management utilization review. Chart data entry, including but not limited to: Physician orders, DME.

- Volunteer Program: maintains compliance of patient contact frequencies through reconciliation of original order; plan of care frequency and volunteer submitted notes; maintains application packets, orientation packets and also volunteer vacations and availability.

- Volunteer Program: Track and maintain active patient and volunteer log to ensure frequency compliance; assist as needed with quarterly mailings and volunteer functions; assist with reports and data entry into EMR software (including Volunteer time).

- Facilitate monthly and quarterly mailings for bereavement, as well as participate in the planning committee for annual service of remembrance.

Qualifications
- High School diploma or equivalent. Associates Degree preferred.

- Healthcare coordination experience preferred.

- Basic understanding of state and federal regulations preferred. Knowledge of EHR systems preferred.

- Skilled to mine data from existing systems and manipulate data to produce reports into a usable format to allow management team to assess the productivity and cost-effectiveness of department operations as well as care delivered. Proficient in computer programs (Excel, Word, PowerPoint, Access, and Outlook) in report writing and analysis capacity.

- Support management on various projects with great results. Highly detail oriented with good organizational skills.

- Goal focused, self-directed and able to work with minimal supervision. Excellent written and verbal communication skills.

- Driving not required.

- Team player that exhibits excellent follow through and a role model for a high standard of professionalism, work ethics and courtesy. Positive inter-personal skills with the ability to work independently and with the Administrative-Quality Team.
