POSITION SUMMARY:

The Annual Wellness Visit (AWV) Registered Nurse (RN) is responsible for conducting comprehensive Annual Wellness Visits in alignment with current guidelines. The RN will assess patients’ medical histories, evaluate health risks, and collaborate with physicians and other healthcare providers to develop personalized prevention plans. This role emphasizes preventative care, patient education, and support, contributing to the overall health and well-being of Community Health Net patients.

EDUCATION AND LICENSURE:

  • Graduate of an accredited nursing program; Bachelor of Science in Nursing (BSN) preferred.
  • Current and unrestricted RN license in the state of employment.

Experience:

  • Minimum of 2 years of nursing experience, preferably in primary care, geriatrics, or preventative health.
  • Familiarity with current Annual Wellness Visit guidelines and requirements preferred.

Skills and Competencies:

  • Strong clinical assessment and patient education skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently under the direct supervision of a physician.
  • Proficient in electronic medical records (EMR) and documentation.
  • Organizational skills to manage multiple patients and meet program goals.
  • Current and valid CPR.

Responsibilities

Patient Assessment and Evaluation:

  • Conduct thorough reviews of medical and family histories.
  • Evaluate current medications and supplements for potential risks and interactions.
  • Measure and record vital signs, including blood pressure, heart rate, and weight.
  • Screen for potential health risks, including depression, cognitive impairment, and fall risks.

Personalized Prevention Plan Development:

  • Collaborate with patients to identify health goals and create individualized prevention plans.
  • Provide patient education on lifestyle factors, such as diet, exercise, and tobacco cessation.
  • Develop recommendations for appropriate preventive services or screenings.

Coordination of Care:

  • Work closely with supervising physician to ensure accuracy and quality of care.
  • Facilitate referrals to specialists or other healthcare providers as needed.
  • Document all findings and plans in electronic medical records (EMR) and obtain physician sign-off.

Program Support:

  • Assist in maintaining compliance with current guidelines for AWVs.
  • Monitor and track patient progress and participation in preventative care programs.
  • Provide feedback and recommendations for improving AWV processes and outcomes.
