Description The AWV Specialist plays a critical role in our clinics as they work closely with the Physicians and medical staff to perform Annual Wellness Visits with our Medicare population. Specifically, the AWV Specialist completes guided steps under the supervision of local healthcare team members to complete key elements of the Annual Wellness Visit to identify gaps in care, safety concerns, and social needs. The AWV Specialist also prepares the patient's chart and communicates any patient needs/concerns to the Providers. Lastly, the Specialist will help conduct outreach when necessary to eligible patients who have not yet completed their AWV. Requirements Job Requirements Maintain patient confidentiality in accordance with HIPAA regulations Attend regular training sessions to stay up-to-date on best practices for conducting Annual Wellness Visits and identifying patient needs Collaborate with the healthcare team to develop personalized care plans for patients based on the findings from their Annual Wellness Visit Utilize electronic health record (EHR) systems to document patient information accurately and efficiently Demonstrate empathy and active listening skills when communicating with patients to build trust and rapport Assist with patient education on preventive care, healthy lifestyle choices, and disease management Participate in quality improvement initiatives to enhance the patient experience and optimize clinical outcomes Maintain a professional and positive attitude while working in a fast-paced, team-oriented environmen Other duties as assigned Requirements High School Diploma Healthcare experience preferred but not required Strong organization skills Outstanding communication and interpersonal abilities Friendly and patient personality Comfortable speaking on the phone Comfortable with computer programs and typing Thrives in a high paced environment Complete OSHA Bloodborne Pathogens training upon hire and annually thereafter Adhere to OSHA standards for personal protective equipment (PPE) and proper hand hygiene protocols Key physical and mental requirements: Ability to lift up to 50 pounds Ability to push or pull heavy objects using up to 50 pounds of force Ability to sit for extended periods of time Ability to stand for extended periods of time Ability to use fine motor skills to operate office equipment and/or machinery Ability to receive and comprehend instructions verbally and/or in writing Ability to use logical reasoning for simple and complex problem solving FLSA Classification: Non-exempt Southeast Primary Care Partners is an Equal Opportunity Employer.
