Key Responsibilities: Lead the planning, deliver and evaluation of the Health Inclusion Project across the PCN and wider community settings. Provide high-quality, autonomous clinical care including assessment, diagnosis, treatment and prescribing (where qualified). Engage with patients in various outreach location- such as hostels, shelters, community centres and mobile clinics, to deliver responsive and flexible care. Work closely with partner organisations including mental health services, housing support, substance misuse services and voluntary sector agencies.
Identify gaps in access or provision and advocate for service improvements and innovation. Educate and support colleagues on inclusion health principles and trauma-informed approaches. Monitor clinical outcomes, contribute to service audits, and support quality improvement initiatives. Promote a culture of compassion, collaboration and equity throughout the PC
