Surrey Place is accredited with Exemplary Standing by Accreditation Canada through the healthcare program.  Position Description Developmental Services Ontario Toronto Region (DSO TR) is seeking an Application Assessor. DSO TR is the access point to services and supports for families in the Toronto Region. It is one of 9 regions of DSO whose supports are based upon the legislated principles of citizenship, fairness and equity, accessibility and portability, safety and security, accountability, and sustainability. DSOs will identify those eligible for developmental services and supports. It will help to assess a person's level of need by utilizing standardized measures and will assist families and individuals in understanding the developmental services system in Toronto and what is available for those individuals. This position reports to the Supervisor, Application Assessors. Main Responsibilities * Completion of the MCCSS Application Assessor training and regular re-certification. * Complete the Application Package for individuals who are eligible for services and supports for Developmental Services Ontario Toronto Region. * Data entry of the Application Package and generation of a report for each individual. * Provide feedback to individuals and appropriate supports/services regarding the individual’s support needs. * Confirm intake demographics for DSO including confirming eligibility and following consent requirements. * Linking to urgent or crisis response, as required. * Community outreach and waitlist support. * Communication and ongoing education including: newsletters; websites; face to face information provision. * Referrals to appropriate services and supports. Qualifications * Completed formal education of an undergraduate degree. * 5 years’ work experience in the field of adult developmental services. * Proficiency with CRM and DSCIS systems, training will be provided. * Excellent communication skills and interpersonal skills. * Regular and consistent attendance at work. * Proficient with Microsoft Office. * The successful candidate must provide an acceptable Police Records Check (Vulnerable Sector Screening) * Fluency in French is considered an asset.
