We’re looking for a proactive and empathetic Referral Coordinator to join our Single Point of Access team within the Durham Mental Wellbeing Alliance to help ensure individuals are placed in the right services at the right time. What you’ll do Receive and triage referrals into the Alliance from a range of sources including health services, social care and housing providers. Gather further information from agencies and individuals to support the referral process and provide high-quality customer service and administrative support across the team. Make informed decisions on suitability and the most appropriate services to meet individual needs. Maintain accurate records of referral outcomes, support needs and risk assessments. Liaise with internal teams and external agencies to ensure smooth referral pathways. Promote our services to maximise referrals and build strong relationships with stakeholders. You have Ideally you have GCSEs in English and maths, but don’t worry if not, you will complete these as part of your apprenticeship learning. You meet the entry requirements of the training requirements which are: to have a fantastic attitude, be keen to learn and build brilliant team relationships We’re a high-tech organisation so we need you to have good IT skills. A genuine interest in learning and personal and professional development – this is a learning role after all! Good communication skills including handling sensitive information with professionalism. You’ll gain stacks of experience to help you launch your career. Many of our apprentices secure a permanent role with us so who knows where you’ll end up in the future! This is a hybrid role, based at least two days a week from our office at Mercury House, Durham and you will be able to work from home on other days. You will have built in time (20%25 of your working week) for study and support for working towards your qualification. You’ll need an Enhanced DBS check done and we pay for that.
