Job Details Job Location: Jawonio Inc - NEW CITY, NY 10956 Position Type: Contractor Salary Range: $1.00 - $1.00 Hourly Wage Range: $52 per visit and $130 per evaluation JOB SUMMARY: To perform physical therapy services to children and adult consumers. MAIN DUTIES AND RESPONSIBILITIES: Essential Elements 1. Performs a broad range of medically prescribed physical therapy programs. Plans program for each consumer, including infants, toddlers, preschool, school age children and adults of all ages as part of home care, out patient and residential services category. Performs a broad variety of exercises based upon evaluations, using physical means such as exercise, massage, heat, water, etc., as prescribed by physician and as appropriate for therapy plan. 2. Prepares various records and documentation of treatments, consumer progress and reactions, and other details as appropriate. Prepares evaluations and recommendations for ongoing treatment. May communicate verbally or in writing to other health professionals, family members, and others regarding status of consumer, justification for equipment orders, and other details related to ongoing treatment plan. 3. Discusses with and communicates to other agency professionals specific situations regarding consumers and to advise and plan for further therapy and assignment of consumers as part of their relationship with the agency. 4. Provides direction and work leadership activities to physical therapy assistants and aides in the carrying out of assigned and routine support functions. May train, coach, and provide motivation and leadership to those assigned in the carrying out of their functions and to broaden their abilities and skills. 5. Oversees the general maintenance of the physical therapy area, making minor adjustments and repairs to equipment such as wheel chairs, and other devices as may be possible. Works with and instructs other agency personnel including residential care staff to implement exercise programs; works with teachers and other staff to incorporate physical therapy goals into their specific programs. 6. May organize clinics for various categories of consumers, setting up administrative and other details for the running of the program, including computer logging, etc. Follows through and tracks attendance to ensure repeats, and performs other functions relating to the ongoing clinic including determination and obtaining of funding. 7. Participates in professional staff meetings, including consultation and discussion with physicians for purposes of developing new therapy paths and to help plan future treatment protocols. May attend goal meetings with school board committees and parents as appropriate. Nonessential Elements 8. May assist in the straightening up and cleaning of facilities and clinic areas. 9. May prepare various clerical or administrative records related to the activity of the department. 10. May carry out in-service programs for agency staff. Maintains safe, orderly work area and work practices. Performs other duties as required #IND1099 Qualifications BACKGROUND/FORMAL EDUCATION/EXPERIENCE: Graduate of approved/accredited physical therapy program, at least Bachelors level degree. INTERRELATIONSHIPS: Important continual contact with consumer and agency professionals. Important but less frequent contact with family and professionals outside the agency such as school district teachers. WORK ENVIRONMENT: Lifting, pushing and pulling of equipment and consumer for most of day. May be required to kneel or bend for periods of time, dependent upon the consumer's individual needs. SPECIAL REQUIREMENTS: Valid NYS licensure for physical therapy.
Responsibilities
