Our philosophy is that every person should be afforded dignity and respect and that no one should be without a safe, affordable, and secure home. We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.
About the role
As an Assertive Outreach Case Manager, this is your opportunity to provide assertive outreach and case management to clients experiencing homelessness, and support to those in longer term housing, who have experienced long term homelessness.
Reporting to the Toward Home Team Leader, this is a permanent, full-time position, based in Adelaide, SA.
The role is in accordance with the SCHADS Award, level 5.
Some out of hours work will be required, which will be rostered. This role will be required to travel to multiple sites as well as to centres within the Toward Home Alliance region.
