Why this role matters
The Salvation Army Australia’s Homelessness Stream provides high-quality, person-centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.
The Salvation Army’s ‘Street to Home’ program provides flexible support to people who are rough or street sleeping, and at risk of primary homelessness.
About the role
We are seeking a values-driven and proactive Assertive Outreach Worker to work as part of a team with people (out of office) who are experiencing primary homeless, rough or street sleeping, and at risk of primary homelessness, to develop and implement case plans that will facilitate their transition to supported accommodation or independent living.
Reporting to the Manager, this is a permanent, part-time (30.4 hours per week) position based in Launceston,Tasmania. Salary and conditions are in accordance with the SCHADS Award Level 5.

You will successfully

  • Work as part of a team with the support of the other Street to Home Outreach workers.
  • Establish effective relationships with rough and street sleepers, and people at risk of primary homelessness to encourage self-determination and social and economic participation.
  • Provide assertive outreach and support to clients.
  • Provide ongoing assessment of clients’ housing and health issues, needs and circumstances from initial intake to exit.
  • Develop individual case plans in collaboration with clients that includes a range of interventions that address the needs and goals of clients.
  • Assist clients to access appropriate support services and establish community supports, and where needed ensuring clients are assisted through advocacy with service providers.

You will have

  • A tertiary qualification in social work, community welfare, community development or a related field.
  • A current Working with Vulnerable People check.
  • A current Tasmanian Driver Licence.
  • Experience and expertise in the areas of Primary Homelessness, Poverty, Social isolation, Domestic Violence, Mental Health, general health, people exiting prison, Disabilities, Youth, Legal issues and AOD.
