Our services have a strong focus on prevention and early intervention and enable access to safe accommodation options, Holistic case management, community outreach programs and programs that teach vital independent living skills. About the role As an Assertive Outreach Worker, this is your opportunity to provide holistic support to young people who are marginalised, excluded and/or disconnected from service and build trust with young people through consistent and meaningful engagement and linking to housing, health, employment, and community services. Reporting to the Program Manager, this is a permanent, full-time position based in Katherine, NT. Salary and conditions are in accordance with the SCHADS Award, Level 4. How you will make an impact Support is provided to young people in line with Youth Services Model of care and associated Frameworks Effective relationships are established with clients that encourage self-determination and participation Case management support is provided to clients in accordance with TSA case management models and standards of practice Ongoing assessment is made of the client’s issues, safety, needs and circumstances from initial intake to exit Ensure the safety and wellbeing of young people and operate within the organisational safeguarding policies and a state mandatory reporting requirement. What you will bring Degree qualification in social work, welfare, community development or related field. Demonstrated high level of sector knowledge and skills attained through previous roles, service and study/development.
