Responsibilities

The Assessment and Transition Coordinator is responsible for ensuring that the prison complies with requirements as outlined within the prison’s Service Delivery Outcomes and other relevant policies.

Some of your duties will include:

  • Participating in the development, coordination and delivery of prisoner risk and needs assessments, parole applications and pre-release services within a prison location
  • Contributing to the Case Management Review process
  • Coordinating and facilitating delivery of sessions and services, both individually and in conjunction with service delivery providers, including co-facilitation with stakeholders and partners.
