Description Position Summary The Assessment Coordinator is a key member of the clinical team responsible for completing mental health and substance use assessments, determining appropriate levels of care, and developing initial individualized treatment plans. This role works closely with the Clinical Director and multidisciplinary team to ensure a smooth admission and referral process for clients and families. Primary Responsibilities Conduct comprehensive mental health and substance use assessments Determine diagnosis and ASAM level of care Develop initial individualized treatment plans Complete required ASAM and admission documentation Coordinate referrals, screenings, and discharges to ensure continuity of care Maintain timely and accurate clinical documentation Collaborate with multidisciplinary clinical team Facilitate psycho-educational groups as needed Physical & Work Requirements Office and facility-based work with frequent interaction with clients and families Ability to manage emotionally challenging situations with professionalism and compassion Requirements Qualifications Minimum 1 year experience in substance use treatment with co-occurring disorders Knowledge of ASAM Criteria and behavioral health practices Strong written and verbal communication skills Ability to work independently and as part of a team Proficiency with EMR systems and basic computer skills Fingerprint Clearance Card (or ability to obtain) Valid Driver’s License with clean record CPR/First Aid certified (or ability to obtain) Bilingual preferred
