We are now seeking an Assessment Worker to join our Hull team — someone who brings curiosity, empathy and confidence when working with people with complex needs, and who understands the importance of timely, high-quality assessment in improving outcomes. Working in Hull In Hull, Change Grow Live delivers assessment and treatment services as part of an integrated local system, working closely with clinical teams, health services, the local authority and voluntary sector partners. Our Hull services are designed to support timely access into treatment, reduce barriers at the point of referral and ensure people are directed to the most appropriate pathway based on need and risk. The Assessment Worker role is central to this model, supporting people from first contact through high-quality screening, triage and referral, while maintaining strong safeguarding, data quality and partnership communication. This work directly contributes to improving local access, strengthening pathways and delivering evidence-based, compassionate support rooted in the Hull community. About the Role As an Assessment Worker, you will be a key point of access into structured treatment services. You will ensure people receive the right support, at the right time, informed by evidence, best practice and individual need. No two days are the same. You will balance engagement, assessment, partnership working and safeguarding responsibilities, using strong judgement and professional curiosity to support people safely and effectively into treatment. Key responsibilities include: Engaging people affected by substance use and providing advice, support and recovery-focused interventions to enable timely access into treatment. Completing high-quality, personalised assessments using appropriate screening tools to identify needs, risks and the most effective pathways into care. Booking nurse-led alcohol assessments where clinical input is required and working closely with clinical colleagues. Liaising with referring agencies and partners to provide updates on engagement and outcomes. Delivering robust harm-reduction advice and brief interventions at triage stage. Managing referral systems and documentation, ensuring accurate and timely record-keeping. Identifying, responding to and escalating safeguarding concerns in line with policy and best practice. Contributing to service performance and access targets, supporting equitable access for priority and treatment-naive populations. Role Details Hours: 37.5 hours per weekSalary: £27,861.26 – £32,002.35 (full-time equivalent) Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. Responsibilities About You You will bring: Experience and understanding of substance misuse and related support needs. Strong assessment, engagement and communication skills, with confidence working with people who may be anxious, ambivalent or in crisis. Excellent written and verbal communication skills, with good IT capability and attention to detail. The ability to manage competing priorities, work autonomously and use sound judgement. Knowledge of mental health services, interventions and good practice. A collaborative, partnership-focused approach, working effectively with internal teams and external agencies. A strong commitment to safeguarding, information governance and safe practice. A willingness to learn, reflect and continually improve your practice. Flexibility, including occasional late working where required. A driving licence is desirable, as there may be occasions where home visits are required.
