Job Description: The Assessor Coordinator will assist the client and family to obtain a high quality of care by jointly developing, implementing and evaluating a plan of care that meets the needs of the client and their family/supporters as the client leaves the hospital to continue with care or to support the client to remain independent in the home setting.

EXPERIENCE

• One year of home care experience preferred

EDUCATION

• Bachelor of Science in Nursing (BScN)

OTHER EDUCATION AND TRAINING

• Practical Nursing diploma is acceptable
• Bachelor of Psychiatric Nursing is acceptable
• Psychiatric Nursing diploma is acceptable
• Nursing diploma is acceptable

KNOWLEDGE AND ABILITIES

• Knowledge of the RAI-HC assessment tool
• Ability to adapt to changing conditions
