Case management is provided to clients receiving services from Home Care, long term care and residents in personal care homes. Core functions include referral/intake, engagement, assessment, care planning, monitoring, evaluation, termination/discharge, through collaboration with family, care givers, and multi-disciplinary team members. This position liaises with other area and provincial services. This position collaborates with acute care, rehabilitation, home care, long term care, mental health, government and non-government agencies to assess clients' needs and coordinate services. Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement. Education: Recognized degree in nursing or social work. Must have a current practicing registration with Saskatchewan Association of Social Workers or the College of Registered Nurses of Saskatchewan (CRNS). Experience Eighteen (18) months of recognized clinical experience (postgraduate) preferred. Knowledge, Skills and Abilities: Theories, concepts and intervention strategies key for case management practice. Knowledge specific to populations being served, for example: Gerontology and Aging. Cost conscious, quality driven care planning and service delivery. Community resources internal and external to the RQHR. Client confidentiality and the Health Information Protection Act. Policies, Procedures, Regulations and Legislation pertaining to area of practice, for example: Mental Health Act, Child and Family Services Act, Health Care Directive and Substitute Decision Maker Act, Youth Drug and Detoxification Act, Community Care Branch Ministry of Health PCH, Home Care legislation and RQHR policies. Demonstrated ability to practice core functions of case management. Openness to teaching and providing educational opportunities. Empathy and respect necessary to engage, develop rapport and build trusting relationships. Effective interpersonal relationship skills and oral and written communication skills. Advocacy, negotiation, crisis management and conflict resolution. Adhering to professional code of ethics and practice standards. Strong team player with ability to build and maintain stakeholder relationships. Standardized documentation which accurately reflects clinical practice. Problem-solving and decision-making skills. Time and Organizational management. Manage caseload and work demands. Facilitate case/family conferences. Technical skills necessary to perform functions of position. HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria. This is a community based position providing service in clinic and home setting. Travel required to various community sites including rural within Primary Health Care networks (Regina). Home base for this position is: 1504 Albert Street.
