This role is patient centered, flexible, cost effective, and quality driven, focusing on advocating for clients and coordinating resources based on assessed needs. Working in Primary Health Care, the position serves as an entry point for accessing services such as community, home, long term, and acute care. Key duties include intake, engagement, care planning, monitoring, evaluation, and discharge. The role collaborates with healthcare providers, families, caregivers, and agencies to assess needs, coordinate services, and ensure timely, appropriate care delivery. Human Resources Exemption: No Education * Bachelor's degree in a related Health Science or Social Science discipline Knowledge and Abilities * Has demonstrated strong interpersonal and communication skills. * Ability to appropriately intervene/attain intervention in emergency situations * Ability to facilitate applications/participate in integrative case management * Ability to develop educational resources and presentations * Demonstrates ability to manage time and competing deadlines * Demonstrates empathy, trust and respect in the provision of care * Knowledge of social determinants of health affecting vulnerable populations * Advocacy, negotiation, crisis management and conflict resolution * Commitment to quality, safety, and improvement striving towards zero harm * knowledge of information privacy principles (LA FOIP and HIPA) * Responsible for accurate observation/documentation of patient related info. * Demonstrated knowledge and ability for competent case management practice * Sound clinical judgement skills. * Adherence to the Code of Ethics. * Ability to establish and maintain interpersonal/consultative relationships  * Cost conscious, quality driven care planning and service delivery * Ability to manage a high demand caseload and other work assignments.
