This role is patient centered, flexible, cost effective, and quality driven, focusing on advocating for clients and coordinating resources based on assessed needs. Working in Primary Health Care, the position serves as an entry point for accessing services such as community, home, long term, and acute care. Key duties include intake, engagement, care planning, monitoring, evaluation, and discharge. The role collaborates with healthcare providers, families, caregivers, and agencies to assess needs, coordinate services, and ensure timely, appropriate care delivery.
