The Intake Facilitator provides screening, triage, assessments for urgency/risk, and coordinates case assignments. The position facilitates crisis response and short-term support for services to clients presenting with a variety of mental health and addictions issues. The Intake Facilitator provides consultation to Community Agencies and Programs regarding services available in Mental Health and Addiction Services.
