Position #: 9008108 Expected Start Date: January 5, 2026 Union: HSAS Facility: Melfort Home Care City/Town: Melfort – NE 5,7,8 Department: Home Care - Case Management Type: Full-time regular FTE: 1 Hours of Work: 224.00 hours per 6 weeks Relief: No Float: No Field Hours: Yes Salary or Pay Band: Pay Band Assessor Coordinator - Degree Senior $39.658 to $48.494 Travel Required: Yes Job Description: Under the direction of the Manager and in partnership with local Network Leaders, the Assessor Coordinator – Degree – Senior is an integral part of operations responsible for putting strategy into action through education, implementation, clinical support and evaluation of the home care services provided by interdisciplinary care teams. They will also have a provincial role in consultation, development, and standardization of home care programming across the Saskatchewan Health Authority. The Assessor Coordinator – Degree – Senior is the clinical and program resource for all Home Care programming needs. They will collaborate with other portfolios with the SHA to support Primary Health Care/Health Service Managers in ensuring front line staff have all the resources, training, and clinical support they require to do their work consistently, safely, effectively, and efficiently, aligning with current best practice standards. Human Resources Exemption: No Experience Minimum two (2) years previous clinical experience in a home care setting Education Bachelor of Social Work (BSW) Licenses Valid Class 5 driver's license Other Education and Training Bachelor of Occupational Therapy is acceptable Undergraduate degree in Health or Social Sciences acceptable to the employer Bachelor of Physical Therapy is acceptable Bachelor of Nursing is acceptable Competencies Data analysis skills Advanced - Computer skills Demonstrates leadership skills Leadership skills such as engagement, communication, and team building Knowledge and Abilities Demonstrated ability in program development, implementation and evaluation Makes judgements on the basis of critical thinking and problem solving criteria Ability to establish and maintain interpersonal/consultative relationships Ability to work with minimal supervision Articulates theory and knowledge related to patient and family centered care Ability to apply research to clinical practice Ability to develop and implement programs / resources from needs assessments Demonstrates ability to accept and implement change Ability to be self directed and work relatively independently Knowledge of Occupational Health and Safety Standards and application of such Demonstrates effective verbal and written communication skills.
