Description Job Purpose The Admissions & Family Support Services Coordinator (AFSS Coordinator) collaborates with members of VIA’s School Leadership to oversee admissions, support successful student transitions, and coordinate comprehensive family support services. This position manages the admissions process from inquiry through enrollment, facilitates discharge and transition planning (including transition to adult services), and serves as a liaison between families, school districts, community agencies, and internal VIA programs. The AFSS Coordinator also provides education, resource navigation, and outreach to promote awareness of VIA’s school, adult, and pediatric ABA services. Duties and Responsibilities Admissions · Serve as primary point of contact for school district partners regarding referrals, admissions updates, and barrier resolution. · Conduct tours for prospective families and school district representatives. · Coordinate intake assessments with families and clinical teams. · Ensure completion and accuracy of enrollment documentation in collaboration with Administrative Assistants and Registered Nurses. · Maintain and monitor admissions applications and documentation systems. · Facilitate new parent orientation and support family transition into assigned VIA school placement. · Track admissions data and support continuous improvement of the enrollment process. Transition & Discharge · In collaboration with VIA school leadership, IEP team, and partnering school division coordinate transition and discharge planning to ensure smooth and well-supported student movement to next placements. · Provide families with education and guidance regarding guardianship, DBHDS waivers, DARS services, and community-based adult services. · Beginning at age 14, collaborate with families and support the IEP team in exploring adult service options within VIA and the broader community, with transition planning and coordination intensifying as the students approach age 22. · Support transition planning for graduates entering adult services, employment, or other post-school placements. Support Services · Serve as a resource to families by providing responsive guidance, resource navigation, and individualized support throughout the student lifecycle. · Collaborate with school teams to address family support needs, participate in IEP planning, and respond to student welfare concerns. · Build and sustain strategic partnerships with community agencies, medical providers, and regional/state resource organizations to strengthen coordinated care. · Support efforts to coordinate, and deliver family engagement initiatives including parent education, support groups, respite programming, and student enrichment opportunities. · Continuously evaluate and enhance family support services to ensure alignment with best practices and evolving family needs. Position Type This is a full-time position that is based in Roanoke. Travel across VIA locations is required. Requirements Qualifications · Master’s degree in Social Work. · Active clinical licensure in Social Work required. · Minimum 2–3 years of experience in human services or related field. · Experience working with individuals with autism spectrum disorder and other developmental disabilities and their families strongly preferred. · Valid driver’s license required.
