Job Introduction
Are you passionate about supporting unpaid carers and making a meaningful impact in your community? We’re offering two exciting full-time opportunities to join our dedicated Carers Network Team as Assistant Care Coordinators, based at the welcoming Brett Young Carers Hub in Halesowen.
These are community-based roles with a strong focus on direct support, assessment, and engagement. You’ll be part of a dynamic team working across the borough to empower carers and connect them to the services they need.
As an Assistant Care Coordinator, you’ll play a vital role in supporting carers over the age of 18, providing tailored advice, emotional support, and practical assistance. You’ll carry out statutory carers assessments and social care assessments under the Care Act 2014, ensuring carers’ needs are identified and met in a timely and person-centred way.
You’ll also support our front-of-house duty function, helping to triage and screen referrals, respond to initial enquiries, and ensure carers are directed to the most appropriate support.
