Job Details Job Location: Central HQ - Las Vegas, NV Position Type: Full Time Salary Range: Undisclosed Job Shift: Mon - Th 7:30am to 5pm & Fri 8am to 12 n Job Category: Health Care Description Position Summary The Assistant Care Navigator (ACN) is a vital link between KSOSN, partner facilities, dialysis organizations, transplant centers, and surgeons. In this role, you'll guide patients through the often-complex processes of participating in value-based care, starting dialysis, and navigating the transplant listing journey. We're looking for someone committed to fostering a high-quality environment, enhancing patient experience and safety, and contributing to an exceptional work atmosphere for both our clinical team and patients. The ACN directly supports and assists Care Navigators with all assigned responsibilities. Essential Job Functions The ACN supports various critical areas of care navigation: Patient Coordination & Support: Assist with daily hospital admissions and support patient activation measures and comprehensive health evaluations. Follow up with patients lost to follow-up. Facilitate access placements (e.g., vein mapping, vascular surgeon referrals, fistula creation, peritoneal dialysis catheter insertion). Coordinate dialysis placements (e.g., peritoneal dialysis, in-center requests from clinics and hospitals, end-of-month dialysis census updates). Support remote patient monitoring (RPM) initiatives. Education & Engagement: Obtain Kidney Smart referrals from the Electronic Medical Record (EMR) and refer patients to Kidney Smart or Kidney Disease Education (KDE) programs. Track patient engagement with education programs (showed, canceled, no-showed) and document enrollment for metrics. Data & Records Management: Track and record key metrics. Request and import patient records from long-term acute care hospitals (LTACHs) into patient charts, and update corresponding spreadsheets. Add dialysis patients to the EMR and scan dialysis personal information forms. Maintain the dialysis census and reconcile monthly dialysis reports. Disclaimer This job description is not exhaustive. Additional tasks may be assigned, or the scope of the job may change based on business needs. Qualifications Minimum Requirements: High School Diploma or equivalent required. Minimum of three (3) years of clerical, administrative or related experience with extensive use of spreadsheets. Minimum one (1) year of related experience in a fast-paced healthcare environment. Must have intermediate knowledge of Microsoft Office applications, especially Excel. Must possess excellent customer service, communication, organizational and interpersonal skills. Must be detail oriented with data entry proficiency. Must have the ability to adapt to change and support new policies and procedures in a positive manner. Must be able to read, write, speak, understand and satisfactorily communicate with others in English in person, over the phone and via email. Associate degree preferred. Medical Assistant Certification preferred. Spanish a plus.
