Description Sangre de Cristo Community Care is looking for a compassionate, experienced Registered Nurse who is ready to grow into clinical leadership. As our Assistant Clinical Manager, you'll partner with the Clinical Manager to support our Hospice and Home Health teams while continuing to make a meaningful impact with patients and families. This is the perfect opportunity for an experienced RN Case Manager or clinical leader looking to expand their leadership skills in a collaborative, mission-driven environment. What You'll Do As the Assistant Clinical Manager, you'll help ensure our clinical teams have the leadership and support they need to provide exceptional patient care. Responsibilities include: Partner with the Clinical Manager in the daily oversight of Hospice and Home Health clinical operations Provide clinical guidance, mentoring, and support to nurses and interdisciplinary team members Assist with staff orientation, onboarding, and ongoing competency development Help coordinate clinician schedules and patient assignments to ensure quality care and efficient operations Participate in interdisciplinary team meetings and case conferences Monitor documentation quality, regulatory compliance, and clinical best practices Conduct chart audits and assist with quality improvement initiatives Serve as a resource for complex clinical situations and patient care concerns Maintain a reduced patient caseload while supporting leadership responsibilities Assume delegated leadership responsibilities in the absence of the Clinical Manager What We Offer At Sangre de Cristo Community Care, you'll join an organization where compassion, teamwork, and excellence are at the heart of everything we do. Requirements Current Colorado Registered Nurse (RN) license Minimum of three years of Hospice and/or Home Health experience Strong clinical assessment and critical thinking skills Excellent communication, organizational, and leadership abilities Previous supervisory, charge nurse, preceptor, or leadership experience preferred Knowledge of Medicare Conditions of Participation and regulatory requirements preferred Current CPR certification Valid driver's license, reliable transportation, and proof of automobile insurance
