Job Overview We are seeking a dynamic, resident-centered nurse leader to serve as Assistant Director of Health Services RN for our Skyview Memory Care at Aurora on France in Edina, MN. This nurse role partners closely with the Director of Health Services Nurse and the Dimensions Manager. The position plays a key role in strengthening team performance, ensuring quality outcomes, and maintaining operational consistency in a specialized environment. This nurse schedule ncludes; 80 hours per two weeks Day shift Weekends as needed Aurora on France is an innovative senior living community attached to Fairview Southdale Hospital in the heart of Edina, MN. Located conveniently near Hwy. 62 and France Ave. and is on a bus line. Aurora offers a full continuum of care including Transitional Care, Enhanced Care and Assisted Living/Memory Care. Responsibilities Maintains current assessment and documentation of client’s health status. Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures RN Assessment is completed upon admission, within 14-days of admission, and at least every 90 days and when/if there is a change in health condition. Works with client and/or responsible party through collaboration, education and timely communication to ensure appropriate service and placement. Assures that clients have up-to-date service agreements. Implements service agreements with new tenant/residents. Reviews and updates service agreements at least once per year and more frequently when there is a change in service needs. Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST. Maintains clinical charts and progress notes using computerized documentation system. Communicates changes in client’s conditions to medical and other relevant individuals. Coordinates client health services. Facilitates coordination of services with internal and external providers. Assures that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations. Works with client and/or representative(s) to assure delivery of appropriate services and funding.
