JOB SUMMARY:

The Assistant Director of Rehabilitation (Assistant DOR) supports the Director of Rehabilitation in managing and supervising the rehabilitation department, including physical therapy, occupational therapy, and speech-language pathology services. This role combines clinical care with leadership responsibilities to ensure effective service delivery, regulatory compliance, and high-quality outcomes for residents.

QUALIFICATIONS:

  • Licensed Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP) in the state of Minnesota.
  • Minimum of 2 years of clinical experience in a skilled nursing facility or long-term care setting.
  • Previous leadership or supervisory experience preferred.
  • Knowledge of CMS regulations, PDPM, MDS, and therapy documentation standards.
  • Strong organizational, leadership, and communication skills.
  • Proficient in therapy software and electronic health records.

Responsibilities
  • Assist the DOR in daily operations of the rehab department, including scheduling, staff oversight, and clinical coordination.
  • Provide direct patient care within professional scope (PT, OT, or SLP) as needed.
  • Monitor therapy staff performance and support professional development and training initiatives.
  • Ensure accurate, timely documentation and adherence to facility, state, and federal policies.
  • Help manage departmental productivity and ensure appropriate utilization of therapy services.
  • Collaborate with interdisciplinary teams (nursing, social work, dietary, etc.) for resident-centered care planning.
  • Participate in quality assurance programs and audits to ensure clinical excellence and compliance.
  • Serve as acting DOR in their absence.
  • Assist with onboarding, mentoring, and evaluating new or current therapy staff.
  • Foster a positive, collaborative, and professional work environment.
