Description:

Your Opportunity:
AHS has an exciting new opportunity for a Clinical Coordinator position based out of Mackenzie Place/QEII Ambulatory Care Center at the Mackenzie Place Transition Care Unit. The incumbent will work with a large group of stakeholders on opening the new Transition Care Unit. The Transition Care Unit is a 19-bed unit that provides care for clients transitioning from hospital to home, who require additional assessments or convalescence time before moving to a Continuing Care Home or returning to their previous home living environment. Reporting to the Care Manager, the Clinical Coordinator provides supervision and leadership to the Transition Care Unit personnel, supporting the day-to-day operations in the provision of safe, professional, and dignified care. The Clinical Coordinator has a key role in the coordination of resident/client care, collaborating with the Care Community to assess, plan, implement, and evaluate care. We embrace the philosophy of person and family centered care and recognize that each client and family member has unique values, personal history and an equal right to dignity, respect, and to participate fully in their environment. Your success will come from creating partnerships and relationships with clients, families and coworkers that will lead to the best client outcomes and enhance quality of life. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.
