AHS has an exciting new opportunity for a Clinical Coordinator position based out of Mackenzie Place/QEII Ambulatory Care Center at the Mackenzie Place Transition Care Unit. The incumbent will work with a large group of stakeholders on opening the new Transition Care Unit. The Transition Care Unit is a 19-bed unit that provides care for clients transitioning from hospital to home, who require additional assessments or convalescence time before moving to a Continuing Care Home or returning to their previous home living environment. Reporting to the Care Manager, the Clinical Coordinator provides supervision and leadership to the Transition Care Unit personnel, supporting the day-to-day operations in the provision of safe, professional, and dignified care. The Clinical Coordinator has a key role in the coordination of resident/client care, collaborating with the Care Community to assess, plan, implement, and evaluate care. As an Assistant Head Nurse, you will be responsible for staff assignments, patient flow and acting as a role model, clinical resource and advocate for nursing staff. In addition to providing frontline nursing care, as required, you will take a leadership role in the promotion of quality patient care and the implementation of operational priorities. Your in-depth knowledge and skills, advanced judgment and clinical experience, will assist in providing solutions for complex health care issues at all levels with patients, interprofessional teams, administrators and policy makers. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
