Responsibilities
  • In coordination with the Health Services Director, lead the clinical team with a sense of purpose and accountability; utilize metrics and established goals.
  • Oversees the medication management services, including training and audit of medication administration practices, collaboration with prescribing healthcare providers, and pharmacy staff
  • Identifies ongoing needs and services of residents through assessment and service plans utilizing the nursing process
  • Visit prospective and current residents in their homes or healthcare settings as needed to assess their suitability for admission or readmission to the community.
  • Clinical liaison with families, residents, healthcare professionals, and team members, and refers them to the Health Services Director when appropriate.
  • Monitors the quality of work performance of Care Partners, Medication Staff, Licensed Nurses, and reports to the Health Services Director.
  • Ensures that infection control procedures are known and followed by staff. Directs the infection control measures for the community.
  • Works closely with staff and designated agent(s) to identify Resident problems, concerns, and issues; follows up as needed and communicates the findings appropriately.
  • Oversees and/or completes monthly nursing schedule for nursing staff. Participates in the Manager on Duty program as directed by the Executive Director.
  • Ensures the accurate transcription of physician orders into the treatment and medication record.
  • Assists as directed with move-in and orientation of new Residents and designated agent(s).
  • Reviews Resident service records, treatment, and medication books to assure quality of care and documentation in the medical record is within nursing standards.
  • Verifies physician orders, move-in/move-out information, and initiates appropriate action/follow-up.
  • Supervises the ordering of medications from the pharmacy.
